Evolution Care Pathways is an ACC approved supplier of an integrated service that supports people recover from musculoskeletal injuries to shoulder, knee and lower back.

This service includes:

Early diagnosis
Early access to specialist care
Flexible service designed based on your needs
Support throughout your recovery
Support to recover, get back to sport and study as required
No co-payment costs

Who can access the pathway?

Anyone can access the service that has an accepted ACC covered injury for their knee, shoulder or lower back and may require a team approach including surgery or specialist review as part of their recovery.

How ICP Works

What does the service look like?

Initial call to greet you and ask a few questions about your injury
Book you in with a physio for an initial assessment
Allocate an ICP Navigator to guide you to stay on track in your recovery
Development of an individualized recovery plan
Early treatment and support based on your needs and goals
Support you through surgery (if required)
Work with you to achieve your recovery goals

What it Covers

Who will help support me through my recovery?

You will have a designated person called an ICP Navigator to help support you through your recovery journey.

They will help co-ordinate the team that support you.

The team may include:

Physiotherapist
Surgeon
Sport Exercise Medicine Specialist
Occupational Medicine Specialist
Rehabilitation Medicine Physician
Pain management specialists
Vocational specialist
Psychologist

How to get Referred

If you think ICP MSK is the right service for you talk with your physiotherapist, GP or Specialist and ask them to send an email referral with your ACC45 number and any radiology reports to us via ICP@evolutioncare.com
