Evolution Care Pathways is one of ACC’s approved providers of this service.
What is the Integrated Care Pathway (ICP)
Once you are referred to Evolution Care Pathways we check you have the right injury claim with ACC and ensure the pathway is right for you.
The service is fully funded by ACC and wrapped around you. It has input from the following team members (based on your needs):
- Specialist Services (Orthpaedic, Neurosurgeon and Sports and Exercise Medicine)
- ICP Navigation
- Return to work support
- Physiotherapy
- Psychology
- Other health or rehabilitation services based on your individual needs
