ICP-MSK is a fully funded ACC musculoskeletal programme. The programme uses an interdisciplinary team who ensure fast access to treatment and rehabilitation for those who have had a significant lower back, knee or shoulder injury.

Are you a Surgeon, Physiotherapist, GP, Rongoā practitioner, Other allied health professional?

Are you treating someone with a recent lower back, knee or shoulder injury that has significant needs? We would love to hear from you.  Evolution Care Pathways provides the platform for the care team  (ICP Navigator, Physiotherapist, Surgeon, Sports and Exercise Medicine Specialist, Occupational Medicine Specialist, Rehabilitation Medicine Specialist, , Return to Work Specialist and others as required) to support the patient in this service.  The eligibility criteria is as below.

Injuries covered by ICP-MSK (diagnosed or likely to be diagnosed) :

Knee

Knee

  • Fracture of the patella
  • Medial and or lateral meniscal tear (or other internal derangement)
  • Osteochondral fracture
  • Fracture involving the tibial condyle
  • ACL rupture with or without meniscal tear
  • Posterior cruciate ligament rupture
  • Medial and or lateral ligament rupture
  • Post traumatic osteoarthritis
  • Patellar tendon rupture
  • Traumatic patella dislocation

Shoulder

Shoulder

  • Fractured clavicle
  • Fracture humerus
  • AC joint dislocation
  • Fracture glenoid
  • Glenohumeral joint dislocation
  • Post traumatic osteoarthritis
  • Rotator cuff full thickness tear
    • +/- biceps tendon high grade tear
    • +/- traumatic labral tear

Lower back

Lower back

  • Lumbar disc prolapse, or extrusion with radiculopathy
  • Lumbar fracture

Email the referral to icp@evolutioncare.com

Include ICP referral in the subject line, attach your referral letter, the ACC45 or ACC claim number.  Attach other relevant information including any radiology or other investigations.
